We try to keep it simple for both you and the referring clinic/physician. Here is the process: A referral form is completed by your physician online or you are directly referred from the clinic to our website. You book and schedule right from our website (www.thrive-wellness.com/evaluation) You complete payment on our website We send you all links to complete documents prior to the evaluation You attend the appointment online in a secure portal Our psychologist will complete the report and submit to your referring clinic. That's it!

Appointments will last anywhere from 30-60 minutes

We are fast... Most psychological clinics have a 2-3 week turn around... We are not like most clinics...Your physician will have your report in 2-3 business days! Many will have it the same day!

At this time, we do not. Here is why: Most insurance companies have 6-9 month (some are more than a year) lead times...we want to do this MUCH faster. Many insurance companies do not fully cover these elective evaluations, meaning more work for you. Ultimately, even with insurance company reimbursement, we have found that MOST patients pay almost the same price and have significantly longer wait times.

Upon request, we can provide a superbill. A superbill is a document that indicates the cost of the evaluation and what was performed by the mental health professional. You will then be responsible for submitting to your insurance company. While this does not guarantee coverage, many insurance companies will reimburse for the entire amount or partial amounts of the superbill.

This is a common question that we get. First, we understand the impact that surgery can have on mental health. We want to ensure that there is mental health stability prior to performing surgery. Second, surgery requires immense social support and behavioral change. We want to ensure that you are well prepared for the significant changes that are required.

This all depends on your clinic's release policy. For this report, the direct "client" is the referring clinic. Therefore, you will not be sent a copy of the report directly by Thrive Wellness and Performance. You can always request this of your referring clinic, but they have the ultimate authority to release that information upon their policy and discretion.

This will be completed by a licensed psychologist.
